WE HAVE HAD SOME PROBLEMS WITH THE ONLINE
FORMS SO HAVE PUT THEM IN PDF FORMAT FOR YOU TO DOWNLOAD THE FORMS, FILL THEM
OUT NEATLY AND SEND WITH CHECK PAYABLE TO THE ADCA, HAIR SAMPLE ENVELOPES, AND A
SELF ADDRESSED STAMPED ENVELOPE -- REMEMBERING THERE WILL BE 2 SHEETS OF
CERTIFIED RESULTS FOR EACH ANIMAL'S TEST THAT YOU RUN.

Please send your check payable to ADCA along
with your sample, Stamped, self addressed envelope, and application to:

The packets will be hand carried to A&M
for processing.
One stamp will cover 4 pages; remembering you
will get 2 sheets for each test, please put the appropriate postage on your
self-addressed envelope.
The results will be mailed to you in duplicate
(with official stamp) and you, the owner, will be responsible for mailing one
official copy to the ADCA - if desired - and keeping one copy for your records.
